A seven-row table, Monday through Sunday, with columns for book title, pages, minutes, and parent signature. Each row is tall enough for a child's handwriting, or for two short books on the same day. A totals row at the bottom adds up the week.
Above the table is a line for a weekly minutes goal. Below it are two lined boxes: The best thing I read this week and New words I found.
Send one home each Monday and collect it the next. The student fills in the title, pages, and minutes after reading, and an adult signs the last column. The totals row lets the student add up the week before handing it in.
