For students who are ready to write about what they read: four entries per page, each with the date, title, pages, minutes, and lined space for a short summary.

Four entry boxes on one page. Each has a header strip for date, title, pages (from and to), and minutes, then six wide-ruled lines for a summary or response.
After each reading session, the student writes two or three sentences: what happened, what they learned, or what they think will happen next. One page covers four sessions, so most classes use one or two pages a week.
The lines are spaced for second and third graders but work through sixth grade. For a more structured response, pair it with a story map or another graphic organizer.
