Below it: a grading scale (A–F by percentage, or E/S/N for younger kids), an attendance table with days present and absent by quarter, a Comments for the year box, and a signature and date line.
Fill in a column at the end of each quarter, or use just Q2 and Q4 if you work in semesters. Letter grades suit older kids; the Excellent / Satisfactory / Needs improvement scale works better in the early grades. Many kids simply enjoy getting a real report card.
Keep the completed card with your attendance records and curriculum tracker as part of a year-end portfolio. Some states ask for evaluations or progress records; requirements vary, so check your state's homeschool law.
